We are providing three different technical ways of filling out the questionnaires. The most convenient one is to download the biggest file as a pdf. You may fill it out directly with your Adobe Acrobat Reader. It should be up to date (Version 7.0) and can be downloaded or updated for free at the Adobe website. We warmly appreciate this version! In case you fill out the questionnaire with Adobe Acrobat Reader and wish to have a copy of your delivered information you shall press `print form` and print the questionnaire before you press 'submit`.
If you prefer to download a smaller file you may choose the pdf or doc format. If you choose the small pdf-format you have to print this file and fill it out manually. If you choose the doc-format you may fill it out with your Word or Staroffice application and send it back as an attached email file.

Of course, all received data will be kept confidential! The published report will not include any individual information that can be traced back to the source.

Questionnaire: Organic Certification Bodies
Please, submit for every wild collection operator and/or project one questionnaire (e.g. in case you are certifying 3 operators, please, submit three questionnaires). In this context it is important that you provide the name of your certification body and/or your e-mail address.
